1. Click the button labeled “Download Census Template”
   This will download the Excel file titled:  OnCentive - Census.xlsx
2. Open the file and click the “Census” tab at the bottom of the spreadsheet.
3. Fill out the fields in the blue headers for each W-2 employee — including full-time, part-time, and seasonal staff.
   (Contractors and 1099 employees are not eligible.)

You’ll be asked to provide:

• Employee ID
• State & Zip Code
• Marital/Filing Status
• Employment Type
• Pay Frequency
• Average Weekly Hours
• Annual or Hourly Wages

   4.  Need help? Use the “Census Data Dictionary” tab in the spreadsheet for guidance on what each field means.

   5.  Save the completed file, then reply by email and attach your saved version and send with the subject line,  Completed Census Form
